This form is for change in any of the particulars registered at the time of Business Name Registration. By filing this form, the user will be able to change and update particulars under the Business Names Act.

In addition to assisting the Registrar to maintain accurate records: (1) Purchasers of businesses should ensure a Statement of Change or Notice of Cessation is filed as part of the purchase process. (2) Partners of a dissolving partnership who wish to continue using the business name should have the Statement signed by all partners before filing. (3) Changes in partnership should be filed promptly to avoid locating former partners later.
Whenever a change is made in any registered particulars, a Statement must be filed within 14 days. Changes include: change of firm name, change of persons/partners, change of place of business, change of registered office, date of change, change of nature of business, any other change.
